Report to Medicare
Last updated September 18, 2026
The short answer
Report Medicare fraud, billing you did not receive, and card scams
Most Medicare fraud shows up as a charge on your statement for something you never got. The report goes to Medicare itself, or to the HHS Office of Inspector General, and drug and Advantage plans have their own line. Check your statements first, because the claim detail is what makes the report usable.
What they handle
- Charges on your Medicare statement for services you did not receive
- Callers asking for your Medicare number to send a brace, test kit or card
- Providers billing for equipment that never arrived
- Medicare Advantage and drug plan billing fraud
How to file a report
- 1
Compare the dates on your statement
Medicare tells people to check the dates and services on their statements against what actually happened. Anything you cannot account for is what you report.
- 2
Call 1-800-MEDICARE
That is 1-800-633-4227, the main line for reporting suspected fraud on your own Medicare record.
- 3
Or report online to the Inspector General
The HHS Office of Inspector General takes reports at oig.hhs.gov/fraud/report-fraud/. It is the route to use if you would rather submit in writing.
- 4
Use the drug plan line for Advantage or Part D
Fraud involving a Medicare Advantage plan or a drug plan goes to the Investigations Medicare Drug Integrity Contractor on 1-877-772-3379, which spells 1-877-7SAFERX.
What you'll need
- Your Medicare number
- The dates and services from the statement in question
- The provider name and any claim number shown
What to expect
Medicare does not publish a timeline for investigating a report. Guard the number itself in the meantime, since a stolen Medicare number is what makes the next false claim possible.
Frequently asked questions
Someone offered me a free brace if I gave my Medicare number. Is that fraud?
That is the standard approach. The equipment is either never sent or is billed at many times its value, and your number is then on file for more claims. Report the call even if you said no.

Medicare scams
Knowing which scam you were hit by makes the report better. This is how scams impersonating Medicare actually work, what the company will never ask you for, and the warning signs to recognise next time.
